Job Description

We are hiring a full-time Nurse Practitioner, including new grads, for home risk assessments in Altoona, PA.

• Conduct comprehensive assessments of adult and geriatric patients in their homes or community settings.
• Review medical history, medication adherence, social factors, and health risks during visits up to one hour.
• Collaborate with physicians, case managers, and caregivers to improve patient care and close care gaps.
• Work Monday to Friday, 8 am to 5:30 pm, with flexibility, no on-call or weekend hours unless desired.
• Offer a competitive salary ($101,700–$132,800) plus incentives, benefits, and mileage reimbursement.
• Require a Master’s degree or equivalent experience, current RN and NP licensure, board certification, and minimum one year NP experience.
