The MDS Coordinator is either a Registered Nurse or Licensed Practical Nurse who assumes responsibility for the completion of the MDS assessment and related reports. Essential duties include following hospital policy on patient safety, providing quality MDS assessments, establishing positive relationships with patients and families, performing ongoing MDS assessments, collaborating with interdisciplinary teams, documenting patient care activities, communicating pertinent patient information to physicians and team members, and supporting hospital mission and values. Education required is graduation from an accredited school of nursing (RN or LPN). Two years of experience with the geriatric population is preferred. Certification/license requirements include a current LPN or RN license in Missouri or Compact State, Certified MDS Assessment Coordinator preferred or to be completed within 6 months of hire, and American Heart Association Basic Life Support. The role requires considerable mental concentration and physical activities such as lifting up to 35 lbs., walking, standing, and other movements. Working conditions include frequent exposure to infectious and communicable diseases and occasional hostile persons.
