Provides psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis. Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes. Employs counseling skills in order to help patient/families understand, accept and follow recommendations for care and to restore patient to optimum social and health adjustment. Stays up to date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance. Intervenes in crisis situations and effectively applies crisis intervention theory techniques. Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare network. Participates in a monthly or yearly audit or review to ensure adherence to laws and other federal guidelines. Works collaboratively with the patients and patient's social support system to establish and implement a plan. Interviews the patient to complete the initial psychosocial evaluation. Reports suspected cases of child and adult abuse/neglect/exploitation. Minimum Qualifications: Master’s degree in social work, 1 year experience working as a social worker or applicable mental health experience, ability to work effectively with people from a broad range of backgrounds, ability to adhere to the National Association of Social Workers Code of Ethics, knowledge of patient rights and laws relative to those rights, such as HIPAA, knowledge of psychological principles and theories, must possess a high degree of emotional intelligence, and empathy towards others, skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems. Preferred Qualifications: LSW - Licensed Social Worker - State of Pennsylvania. Physical Demands: Lift and carry 7 lbs., continuous sitting >67%, frequent keyboard use/repetitive motion, frequent fine motor activity/wrist position deviation. Work Schedule: Monday-Friday; 12:00 pm - 8:30 pm; weekend and holiday rotation. Work Shift: Day Shift. Position Type: Onsite.
