The Palazzo is a Full-Continuing Care Community offering Independent Living, Assisted Living, Skilled Nursing, and Memory Care. Our programs are created to accommodate a resident's specific needs. Independent Living provides social activities, nutritious meals, housekeeping services, and care as needed. Assisted Living is for those requiring 24/7 care and a personalized care plan. Skilled Nursing is available for residents transitioning from a hospital stay and in need of rehabilitation or more long-term care. Memory Care specializes in loving and compassionate assistance for your loved one who suffers from Alzheimer's/Dementia.
Summary: The LPN/RN supports the Wellness Director in achieving the goals and objectives for the Assisted Living and Memory Care Unit by ensuring high quality clinical services
Qualifications: •Active LPN /RN licensure in the State of Arizona. Must be in good standing
•Minimum of two years of clinical experience required
•Experience in Assisted Living, experience with Dementia and Alzheimer's residents preferred
•Strong interpersonal skills with the ability to communicate effectively with a wide range of audiences, both internal (care staff, nurses, executives, etc.) and external (families, residents, physicians, vendors, etc.)
•Current First Aid and CPR certifications Food Handlers card, finger Print Clearance card
Essential Functions: •Supports the philosophy, mission, commitment, vision, and values of the Company.
•Assist in managing the day-to-day healthcare operations of this AL/ALZ Community to ensure residents' healthcare needs are met
•Provide overall clinical leadership assistance
•Coordinate nursing/resident services to ensure the continuity of the resident's total regimen of care
•Supervise all care staff, including interviewing, evaluating, training, mentoring, assigning work schedules, developing performance improvement plans
•Participate in the development, implementation and enforcement of policies, procedures, and systems
•Assure regulatory compliance at all times
•Participate in Quality Assessment and Performance Improvement Program
•Safety Committee Meeting
•Adhere to the department's budgetary obligations
•Communicate with physicians and family members
•Resolve complaints regarding resident care
•Provide training and supervision to associates in the administration of direct care within the community; provide hands-on resident care as needed or warranted
•Conduct quarterly assessments and develop subsequent service plans
•Perform ongoing nursing assessments/observations of residents' physical and psycho-social needs, and coordinate with other departments to assure quality care
•Provide continuity of care for those residents receiving home health care, hospice services, and other third-party healthcare-related services
•Serve as a point of contact for the resident's family members, vendors and professional contacts in the community, dealing with issues such as scheduling, resident care, service plans, and assessments and marketing Performs other duties as deemed necessary.
•Conducts job responsibilities in accordance with the standards set out in Company's Code of Conduct, Company policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
Knowledge/Skills/Abilities: - Successful experience building relationships that lead to closing sales and collaborating internally to achieve the business goals.
- Successful experience generating interest, overcoming objections, and building positive customer relations.
- Skilled in directing and motivating the workforce.
- Ability to communicate effectively with clients and their family members and with all levels of the organization.
- Ability to react decisively and quickly in emergency situations.
- Ability to organize and prioritize.
- Knowledge of training techniques for clinical staff.
- Ability to maintain confidentiality.
